At Plexus, we see the person (not the billing number) and are here to encourage you to play an active role in your longevity. As a new patient to our practice we want you to feel empowered to make informed and interconnected decisions along your individual health journey. This starts with getting to know you and your personal health history.

Upon booking your first appointment with us, we will ask you to fill out our New Patients form. We also recommend you request your medical records to be transferred to us. With this information we can meet you ready and prepared to discuss your health concerns.

On the day, we suggest you arrive at least 5 minutes before your appointment so we can process any relevant documents before you see your GP. Please ensure you have your Medicare card on hand so we are able to bulk bill you.
